How it works

From your form to an approved report in six steps.

You set up your form once. After that, every report starts with a phone call and ends with your approval.

  1. 01

    Set up your form

Upload the fillable PDF you already use and vForm maps its fields. Paper or scanned form? We build the fillable version for you.

  2. 02

    Get a report number

Your workers get a phone number to call. The Large teams plan can run several numbers with routing.

  3. 03

    Call in

Guided mode asks each question on the form. Dictate mode lets the caller talk it through. Callers are asked for recording consent first.

  4. 04

    The form is filled

Answers are mapped to the fields on your PDF. The call recording and transcript are saved with the report.

  5. 05

    A supervisor reviews it

Correct any field, then approve or reject the report. If you require a caller roster, calls that don't match are flagged.

  6. 06

It goes where it needs to

Download the approved PDF, or send it to your other systems by signed webhook or API.

Two ways to call.

Pick the mode that fits the form. Short, structured forms work well guided; end-of-day reports work well dictated.

Sample guided call
